| REGISTRATION |

In order to provide you with the best possible care, we need the most up-to-date information about you. If you are a new patient, or an established patient who has not visited our office within the past three months, we will need an updated registration form at your next appointment. Please arrive 15 minutes before your scheduled appointment.
When you schedule your first appointment with us, we will mail you a "new patient" package. Please complete the registration form included and bring it to your appointment. Your insurance card and your copay are required at the time of service. We will be happy to answer any and all of your questions at the time of your visit.
